項目摘要
嚴重周圍神經損傷是臨床實踐中的難點,研究發現周圍神經再生存在倍數擴增現象,即遠端再生的神經纖維數量可以大大超過損傷平面近端神經纖維的數量。本實驗室前期工作顯示,在遠端有足夠可長入空間的條件下,周圍神經再生倍數擴增極限的理論值為3.3,受區與供區神經纖維數量的比率影響倍數擴增率與重建效果。本課題將在前期工作的基礎上,進一步探討:1.影響倍數擴增的因素(不同神經平面擴增、二次級聯擴增、二期修復擴增對倍數擴增極限的影響);2.倍數擴增的神經纖維在重建過程中的轉歸;3.遠端神經纖維倍數擴增後脊髓與腦中樞神經功能區數量及再分布變化的重塑規律。為利用倍數擴增效應修復周圍神經損傷中,供區神經的選取,重建方案的科學決策以及修復時機的選擇提供新的思路和實驗依據。
