項目摘要
注意缺陷多動障礙(ADHD)是最常見的兒童精神障礙之一,患病率為3%-6%,該病嚴重影響患者的學業、職業和生活,給其家庭和社會造成沉重的負擔。ADHD的病因及神經病理機制尚不清楚,且異質性強,不同亞型ADHD的臨床表現、共患病、藥物治療反應、結局及預後等均存在差異,其神經病理機制是否相同,目前仍存在爭議。至今國內外尚未見對ADHD不同亞型的多模態腦磁共振成像研究。本研究擬採用多模態腦核磁共振成像技術(包括結構磁共振、靜息態功能磁共振、彌散張量成像和磁共振波譜成像四種模態),選擇未經藥物治療的、且無共患病的ADHD混合型、注意缺陷為主型兒童及正常匹配對照各30例,探討不同亞型ADHD的腦結構和功能差異。磁共振不同模態之間聯合分析可以互相補充、互相印證,以進一步揭示ADHD不同亞型的神經病理機制,為今後ADHD臨床診斷及分類提供客觀依據。
